Two physicians based at the UVM College of medicine have created a company whose software helps both diabetes patients and their doctors track their helath.  Dr Ben Littenberg and Dr Charles MacLean created Vermedx, whose programs embody the Chronic Care Model, a widely embraced approach to improving health outcomes.  It emphasizes enabling provider teams to be prepared and proactive, and to interact regulary and effectively with well informed patients.

Vermedx had its origins with a research project that involved a Vermont Diabetes Information System funded by the National Institutes of Health with a $2 million grant from 2000 to 2005.

After the trial period for diabetes monitoring was found successful in 2005, the software and technology was spun off and marketed. The two principals worked with the technology transfer office at UVM and now have a patent application under review.
